I have attached the directions and My psychosocial part 1 for help on this part 2
This section provides an opportunity to present a brief statement of the psychosocial problem(s),
emphasizing those areas which will be the focus of intervention. The statement should
include the psychosocial factors and conditions causing the problem(s); the evident factors
contributing to the problem(s); and the relevant complicating and/or alleviating factors affecting
problem resolution or reduction. *Material included in the first three sections should be utilized
as a basis for formulating the service plan. The plan should be theoretically sound and relevant to
client problem(s) and goals.
A. Goals
1. One long-term goal and related objective.
2. One short-term goal and related objective.
3. General and specific tasks for long- and short-term goals.
4. Strengths and obstacles
B. Prognosis
1. Anticipated response of client and other participants to the service plan.
2. Length of time the service plan will likely take to achieve and why (consider the Stages
of Change, strengths and obstacles and any other considerations unique to this case).
ADDITIONAL CRITERIA AND GUIDELINES:
Revisions
Students address instructor feedback in revision of Parts I. Part II should reflect an
understanding of feedback on content, concepts, and writing provided by the instructor on Parts I
and II. This means that students submit two final documents, listed below. Failure to do so will
result in an automatic deduction of 2.5 points.
1) A document with track changes made by the student to address feedback in Parts I.
2) A clean document with all track changes accepted.
Emphasis on Evidence-Based Practice
It is imperative that social workers and social work practice draw on a comprehensive
understanding of evidence-based practices (EBP) and guidelines.
*Each psychosocial assessment must be accompanied by at least five distinct scholarly
resources cited in the goal setting and prognosis section that provide guidance in service plan
development. This may encompass best-practice guidelines or actual intervention strategies.
